This simple script allows visitors to send you a "rating" on specific pages on your website to you, via email. There is no need for MySQL to use this script, however it would be simple enough to alter this script to send the ratings to a MySQL database and allow MySQL to "count" the number of awesome, OK, and "it sucks!" ratings you get.
